OBJECTIVE:

 

The primary purpose of your job position is to provide routine nursing care in accordance with our established policies and procedures, and as may be directed by the Unit Charge Nurse, Director of Nursing, or Administrator, to assure that the highest degree of quality resident care can be maintained at all times.

 

GENERAL INFORMATION:

 

Listed below is an outline of the major duties and responsibilities that you will be required to perform. Even though your job description is broad, every effort has been made to make this outline as complete as possible. HOWEVER, we must emphasize that you may be required to perform other related duties and activities.

 

RESPONSIBILITIES:

1. Perform all duties as assigned.

2. Assist feeding of residents. Remove dishes when finished.

3. Perform after meal care as required. (Cleaning residents’ hands, face, clothing, etc.)

4. Assist in preparing residents for meals. (Taking to/from dining room, serving food, placing bibs, assisting in feeding or cutting food, supervision in dining room, etc.)

5. Assure that residents’ food is accessible and self-help devices are available as needed.

6. Feed residents who cannot feed themselves.

7. Serve extra nourishment in accordance with established nursing care procedures.

8. Serve drinking water in accordance with established nursing care procedures.

9. Assist residents in preparing for test, X-rays, therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.

10. Assist in therapy treatment as instructed.

11. Assist residents into chairs, beds, bathroom, bathtubs, showers, etc.

12. Assist residents to walk with or without self-help devices as necessary.

13. Clip and trim fingernails, eyebrows, etc., as directed.

14. Wash wheelchairs, walkers, etc., as directed.

15. Dress residents neatly, and in their own clothing. Assist residents in dressing as necessary.

16. Comb and brush resident’s hair daily. Shampoo as instructed.

17. Shave male residents daily, or as instructed.

18. Keep hair under arms, legs and face on female residents clean shaven as necessary or instructed.

19. Give back care as instructed.

20. Assist residents to/from bathroom. Offer bedfast residents bedpans, urinals, portable toilet, etc. Clean and store such equipment AFTER EACH USE. (Report intake/output as directed)

21. Keep incontinent residents clean and dry as possible at all times. Change bed linens and clothing as often as necessary. Apply incontinence products correctly.

22. Provide cool cloths, foot boards, etc., to the residents as directed.

23. Answer resident calls promptly.

24. Keep blankets on bed to assure that residents are warm at all times.

25. Assist residents with dental and mouth care.

26. Assist residents with identifying food arrangement. (i.e. informing blind residents of where food is located on the tray)

27. Record resident’s food intake as directed.

28. Bathe residents as assigned and in accordance with our established nursing care procedures. (Encourage showers and other self-help measures/activities)

29. Observe and report the presence of pressure areas to prevent bedsores.

30. Check and report bowel movements and character of stools as directed.

31. Collect specimens, stools, sputum, etc., as directed.

32. Place resident in correct and comfortable position and assure that side rails are up.

33. Move and turn bedfast residents at least every two (2) hours and record such information as directed.

34. Observe disoriented and comatose residents as directed. Record data as directed.

35. Report all complaints to charge nurse.

36. Perform restorative and rehabilitative procedures as directed. (i.e., splints)

37. Report any medication found in the resident’s room. Take to the charge nurse as directed.

38. Perform routine nursing care such as changing, slings, colostomy bags and drainage bottles, etc., as directed by the charge nurse.

39. Chart required information as instructed.

 

PRODUCTIVITY AND INITIATIVE:

 

  1. Report on and off duty.
  2. Check residents at least hourly.
  3. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit.
  4. Other related duties as may become necessary or as directed by the charge department director or administrator.

 

 

KNOWLEDGE OF WORK:

 

  1. Receive assignments and check your resident schedule.
  2. Perform all assigned duties in accordance with our established policies and procedures, nursing care procedures, and our safety rules and regulations.
  3. Verify the identification of resident before administering treatment, serving food, etc., to assure that appropriate treatment, etc., is being provided to the correct resident.
  4. Report any resident abuse immediately, (i.e. harsh/abusive language, unnecessary roughness, etc.)
  5. Attend and participate in in-services educational programs, on-the-job training programs, etc., as directed.
  6. Follow infection control procedures, as established by this facility, when isolation precautions are necessary.
  7. Maintain CONFIDENTIALITY of all pertinent resident care information to assure resident rights are protected.
  8. Report all accident/incidents, regardless of how minor, to the charge nurse as soon as possible.

 

COOPERATION:

 

  1. Assist others as needed to maintain quality resident care.
  2. Assist others in heavy lifting. USE PROPER EQUIPMENT.
  3. Ensure that resident’s rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to wage a complaint, are followed at all times.

 

SAFETY/HOUSEKEEPING:

 

  1. Make beds, clean bedside tables, clean bedrails, etc., as well as other housekeeping duties that relate to nursing care procedures.
  2. Use safety devices appropriately, and only when ordered. Check safety devices at least every hour and release restraint every 2 hours for ten minutes. Record such data as directed.
  3. Report any resident leaving the premises, or who has left the premises to the charge nurse immediately. Assist wandering resident from leaving building for his or her safety.
  4. Keep floors dry and free of unnecessary equipment, supplies, etc., to prevent accidents.
  5. Do not permit resident to smoke in bed. Attendant or visitor must be present at all times resident is smoking. Smoking permitted in designated area only.
  6. Assure that established smoking regulations are followed by personnel, visitors, residents, and the general public. Report violations to the charge nurse immediately.
  7. Assure that the nurses call system is attached to the bed when residents are bedfast and within easy reach at all times.
  8. Keep residents’ personal articles off floor and properly stored.
  9. Keep supply room and work areas clean at all times.
  10. Wash hands before and after caring for each resident and at other appropriate times to maintain infection control.
  11. Watch for and report any change in room temperature, ventilation, lighting, etc.
  12. Report all hazardous conditions and equipment to the charge nurse immediately.
  13. Wash wheelchairs, walkers, etc., as directed.
  14. Check for food in residents’ rooms. Discard as outlined within our nursing care procedures.
  15. Assure that work/assignment areas are clean and equipment, tools, supplies, etc., are properly stored before leaving such areas on break, end of workday, etc.

 

 

ATTENDANCE AND PUNCTUALITY:

 

  1. Report on and off duty at break times and meal times.
  2. Report for duty on all scheduled days.
  3. Must arrive on time.

 

WORKING CONDITIONS:

 

  1. Works in well-lighted/ventilated work areas, as well as throughout the nursing care service areas.
  2. Sitting, standing, bending, lifting and moving intermittently during working hours.
  3. Subject to frequent interruptions.
  4. Involved with residents, family members, personnel, visitors, etc., under all conditions/circumstances.

 

  1. Subject to hostile and emotionally upset residents, family members, etc.
  2. Communicates with nursing personnel, and other department supervisors
  3. Willingness to work beyond normal working hours and in other positions temporarily, when necessary
  4. Attend and participate in continuing educational programs as directed.
  5. Subject to falls, burns from equipment, infectious diseases, substances, odors, etc., throughout the work day.
  6. Be economical and careful with supplies, equipment, etc.
  7. Follow all of our established policies and procedures, to include nursing care procedures, safety regulations, personnel policies, etc., to assure that quality resident care can be maintained.

 

QUALIFICATIONS:

 

  1. Education:   Must possess a High School diploma, GED and current PA certification.
  2. Experience: On-the-job training provided.
  3. Specific Requirements:
  4. Must possess a sincere desire to serve the ill, aged, and infirm
  5. Must be able to read, write and speak the English Language
  6. Possess the ability to make independent decisions when circumstances warrants such action
  7. Possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public
  8. Be a minimum of 18 years of age
  9. Willingness to work harmoniously with other personnel
  10. Must have patience, tact, cheerful disposition and enthusiasm, as well as be wiling to handle residents based on whatever maturity level they are currently functioning.
  11. Possess the ability to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

 

PHYSICAL REQUIREMENTS:

 

  1. Must be able to move intermittently throughout the workday.
  2. Must be able to speak the English language in an understandable manner
  3. Must be able to cope with the mental and emotional stress of the position.
  4. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  5. Must function independently, have flexibility, personnel and support agencies
  6. Must be in good general health and demonstrate emotional stability.
  7. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility.
  8. May be required to lift, transfer, move residents, medical equipment, supplies, etc.
